Admission/Discharge Coordinator - Mental Health
This is a full-time position under the direct supervision of the Long-Term Structured Residence (LTSR) Director. The Admission/Discharge Coordinator- Mental Health performs functions to support individuals through the process of admission and discharge at the WLTSR program.

DUTIES AND RESPONSIBILITIES OF THE ADMISSION/DISCHARGE COORDINATOR- MENTAL HEALTH:
- Interview all referrals received for the placement at the WLTSR
- Collect all additional information related to the referrals
- Document and review all information on referrals with WLTSR team consisting of the Director, Coordinators and Psychiatrist
- Document the team’s decision of admission approval on non-approval
- Communicate to all necessary entities (County, referring entity) the outcome of the referral review
- Document the plan and monitor progress
- Review weekly with the WLTSR team including the Director and Coordinators
- Complete monthly Carelon reviews or as required
- Upon admission, assess the needs in regards of benefits for the individual including but not limited to Social Security, Medical Assistance, Identification, Rep Payee, and other financial needs
- Develop a plan to work on obtaining all necessary items
- Attend, participate in each individual’s 305 court hearing to maintain, from a legal perspective, the individual’s ability to stay in a restrictive setting
- Review initial assessments and treatment plans
- Develop a specific and initial plan to access all additional outside services including but not limited to case management, drug and alcohol therapy, counseling and a plan to obtain the services
- After an individual is in the program for one month review all information on progress and participation in the program
- Develop and document and initial discharge plan and any additional needs
- Review with the WLTSR team including the Director and Coordinator for additional items and approval
- Participate in Carelon recovery meetings for individuals as needed
- Participate in weekly meetings with the individuals and the Psychiatrist
- Participate in monthly treatment team meetings
- Work with the individual, team and outside entities on discharge by making all necessary referrals to community programs, ensuring additional follow up care when discharged, trial visits etc..
REQURIED SKILLS AND KNOWLEDGE OF THE ADMISSION/DISCHARGE COORDINATOR- MENTAL HEALTH:
- At a minimum associate’s degree, or 60 credit hours, from an accredited college or university and at least one (1) year experience coordinating admissions and discharges
- Valid Pennsylvania Driver’s License
- An understanding of Mental Health
- $62,500 annual salary
