We are currently looking for a Nutrition and Wellness Manager to fill a full-time position in the Harmar area. The Nutrition and Wellness Manager position is a full-time position under the direct supervision of the Director of Nursing. The Nutrition and Wellness Manager is responsible for assessing nutritional needs of individuals with intellectual and developmental disabilities, developing individualized nutritional and dietary plans, and providing nutrition education and counseling to support overall health, wellness, and management of chronic conditions.
The Nutrition and Wellness Manager will also develop educational materials and provide trainings for staff and individuals on nutrition, healthy eating habits, special diets, and safe food practices. The position will also support physical wellness initiatives, including developing basic fitness or activity plans, providing education on exercise and healthy lifestyle habits, and accompanying individuals to community exercise activities to provide demonstrations and support. The position will support residential program teams with menu planning, therapeutic diets, weight management, and nutrition-related health concerns to improve quality of life and health outcomes for individuals supported by PMH.

DUTIES AND RESPONSIBILITIES OF THE NUTRITION AND WELLNESS MANAGER:
- Conduct comprehensive nutritional assessments for individuals including dietary habits, medical conditions, weight history, and nutrition-related health risks.
- Develop and implement individualized nutritional therapy and dietary plans, including therapeutic and texture-modified diets as needed.
- Provide individual or group nutritional counseling and education to individuals and staff.
- Develop educational materials, resources, and trainings related to nutrition, meal planning, special diets, and safe food practices.
- Provide guidance and consultation to residential program staff regarding menu planning, grocery shopping, and meal preparation.
- Monitor weight changes and nutrition-related health concerns and make recommendations as appropriate.
- Work collaboratively with the program team and medical providers as necessary as part of an interdisciplinary team to coordinate individualized care.
- Maintain accurate, timely documentation of nutritional assessments and interventions.
- Ensure all nutritional services comply with state, federal, and accreditation standards.
- Develop individualized fitness and/or physical activity plans in coordination with the interdisciplinary team.
- Provide education to individuals and staff on the importance of physical activity.
- Accompany individuals to gyms, community fitness centers, or other wellness activities as needed to provide demonstrations, supervision, and support with exercise.
- Perform other duties and responsibilities by the Director of Nursing
REQUIRED SKILLS AND KNOWLEDGE OF THE NUTRITION AND WELLNESS MANAGER:
- Valid Pennsylvania driver’s license.
- Registered Dietitian (RD/RDN) credentialed by the Commission on Dietetic Registration (CDR); must maintain active registration and be able to provide verification.
- Licensed Dietitian Nutritionist (LDN) in the state of Pennsylvania, in good standing.
- Ability to meet applicable Pennsylvania Department of Human Services (DHS) / Office of Developmental Supports (ODS) provider qualification and documentation requirements for licensed professionals.
- Bachelor’s degree in Dietetics, Nutrition or related healthcare field.
- Experience working with individuals with intellectual or developmental disabilities preferred.
- Experience with therapeutic diets (diabetic, heart healthy, low sodium, renal, texture-modified diets, etc.).
- Strong communication, training, and presentation skills.
- Knowledge of basic fitness principles, exercise safety, and physical activity guidelines.
- Ability to safely demonstrate basic exercises and use of fitness equipment. Ability to work independently and as part of an interdisciplinary team.
