We are currently looking for a Program Specialist to fill a full-time position in the Harmar area. This is a full-time position under the direct supervision of the Program Specialist/ Program Operations Director. The Program Manager manages and oversees the daily operations of the 55 Pa. Code Chapter 6400 Community Homes for Individuals with an Intellectual Disability, Dual Diagnosis (Intellectual Disability/Mental Health Diagnosis), and/or Autism Diagnoses. The Program Manager provides supervision, training, support and guidance to the House Coordinator and Direct Support Professionals.

DUTIES AND RESPONSIBILITIES OF THE PROGRAM MANAGER:

  • Promotes the Mission and Philosophy of Passavant Memorial Homes Family of Services.  Acts in accordance with all agency policies.
  • Demonstrates dignity and respect for the consumer’s and their families.
  • Provide ongoing supervision as needed to the House Coordinators and Direct Support Professionals
  • On-call for emergencies
  • Assist with interviewing and training of all Direct Support professionals
  • Completes On-The-Job-Training (OJT) for new employees
  • Complete transfer training for Direct Support Professionals that transfer into the home
  • Coordinate the training of Direct Support Professionals in the content of Health and Safety needs relevant to each individual’s Health Risk Screening Tool (HRST) and IP
  • Assure budgetary compliance and fiscal accountability for the residential site.
  • Complete attendance/payroll records for Direct Support Professionals.
  • Review KRONOS at least bi-weekly to ensure accurate scheduling
  • Complete KRONOS edit sheets as necessary
  • Ensure the appropriate scheduling and staffing of the home
  • Complete medication observations for Direct Support Professionals on a quarterly basis
  • Ensure program compliance with agency policies, state and federal regulations, local monitoring standards, and contractual requirements.
  • Assist with the development and implementation of the Individual Program Plan
  • Attend individual team meetings
  • Ensure all Direct Support Professional’s trainings are up to date and documented through Relias and In-services
  • Ensure implementation of all supports as specified in the person’s individual program plan, personal goals, schedule and support plans.
  • Provide direct support to individuals in the program as needed
  • Coordinates and participates in Interdisciplinary Team meetings
  • Assure time guidelines are followed for completion of IDT information
  • Assist in coordinating, completing and reviewing individual assessments
  • Interact with other departments to meet individual’s needs.
  • Act as agency representative as assigned.
  • Conduct monthly Fire drills
  • Conduct fire drills for new admissions and new staff
  • Ensure efficient supply and grocery procurement
  • Ensure the coordination of annual and follow up medical appointments
  • Oversee and ensure all appropriate documentation is completed by the Direct Support Professionals via electronic MAR, Care Tracker and chart
  • Ensure the monthly review and follow up for all vehicles (fuel logs, vehicle safety inspection)
  • Ensure the regular and effective communication to family, Supports Coordinators and other team members
  • Approve Paid Time Off (PTO) and coverage
  • Ensure accurate handling and documentation of Individual and house petty cash
  • Other responsibilities as assigned by the Program Specialist and Program Operations Director

REQUIRED SKILLS AND KNOWLEDGE OF THE PROGRAM MANAGER:

  • High School Diploma and three (3) years’ experience working directly with individuals with developmental disabilities
  • Valid PA driver’s license
  • Effective verbal and written communication skills
  • Ability to maintain confidentiality
  • Ability to complete work assignments independently
  • Minimum of two years supervisory experience
